This page provides a convenient way for
Oral Health Section staff employees to access forms.

Each of the headings below is a "package" that contains the forms. Click on the heading to expand the list of forms contained in the package, then click on the form you need to download. The form will open in the program it was created in, such as Microsoft Word, Excel, or Acrobat. You can then complete the form in the same way you always have, and either print it or "save as" on your computer for your records, or to send to your supervisor. If you click on the heading of the expanded list, the list will collapse.

Please Note: A list like this that expands and collapses might be seen by some internet browsers as a virus threat, and the browser will try to block it. Usually, a yellow warning bar will appear at the top of the window. If you click on that yellow bar, a dropdown menu will appear that will give you the option to "allow blocked content". If you click on "allow blocked content", the list will work properly.

BEACON Information

BEACON is the name for our new payroll system that will begin on April 1, 2008. You can activate your BEACON account now if you have received your new BEACON card in the mail.

BEACON is an acronym that stands for Building Enterprise Access for North Carolina's Core Operation Needs. By now we hope you have heard about BEACON and how you will have access to your personnel data via the web as long as you have a computer that is hooked up to the internet. You will have the capability to view and change such data as your address, bank account, benefits, printing out your pay stub, just to name a few things you will be able to do.
DHHS has set-up a very good web site to communicate the latest information/changes.

There is also a BEACON Reference Guide for State Employees, a 20 page information guide to help with the transition.


Your new BEACON ID card:

During the month of March 2008, you will receive in the mail at your home address a card that will have your new BEACON identification number (ID). This new BEACON ID number will remain with you throughout your state career employment regardless of which state agency you work for and it is the same number used by our state retirement system, so do not lose this card.



BEACON software Installation:

The IT folks have provided BEACON software that will prepare your computer for interacting with the BEACON web site.

The Beacon software is simple to install.
Click on this NCBeconUser.exe file and save the file to your desktop.
Once the file is downloaded, double click the “NCBeconUser.exe” file.
The Beacon User Installation window opens and the installation begins.
No other intervention is necessary until the installation is complete.
Click OK, and you’re done.

BEACON Overview Training:

During the month of March you will need to complete either one or two (supervisors will complete two) on-line training modules so you will understand the BEACON web based program and can navigate to find and print out your personnel data. This takes about 30 minutes.

BEACON Authorization Procedure:

In order to successfully login to BEACON, all DPH users will have to link their BEACON account to their NCID account. Most of you should be familiar with your NCID account details (username and password) as it is currently required to login to NCMail. This BEACON Authentication Procedure from the IT folks will help you through this process. You can activate your BEACON account now if you have received your new BEACON card in the mail. Do this before April 1st.

 

REVIEW CHECKLIST, What you need to do to prepare for BEACON:

1) Install the BEACON software (NCBeaconUser.exe).
2) Complete the BEACON Overview Training.
3) Memorize your new BEACON ID number; keep the card in a safe place.
4) Before April 1st, complete the Beacon Authorization Procedure to link your current NCID number to your new BEACON account.
